How accurate is this domain check?

We query RDAP — the registries' own live lookup protocol — so results reflect actual registration status, not a cached list. \u201cAvailable\u201d means no registration exists right now; someone could register it an hour later, so if you love a name, register it promptly.

Which domain extension should a local business choose?

.com remains the default customers type and trust; grab it if it exists. .co and .io suit tech brands, .us works for explicitly American businesses, and .net/.org are fine fallbacks with established meaning. If your .com is taken by a parked page, a slightly longer .com usually beats an exotic extension.

The name I want is taken — now what?

Check what's actually there: a parked domain may be buyable through a broker, sometimes for less than you fear.
